NDIS support coordination

The National Disability Insurance Scheme (NDIS) aims to improve the lives of people with disability, their families and carers by providing access to funded supports, information and connection to services. However navigating the NDIS and the wide variety of support services can be daunting and overwhelming. Through Support Coordination services, Carers NSW can assist you to 'optimise' your plan ensuring that you or the person you care for is getting the most out of your funded supports.

What is Support Coordination?

Support Coordination is a service that assists you and the person you care for to:

  • Learn, navigate and better understand the NDIS
  • Gain a greater understanding of the NDIS supports available in the plan
  • Understand how the NDIS plan budget works
  • Connect and coordinate services that best meet your needs and the needs of the person you care for.

How can we help?

Carers NSW Support Coordinators can help people with a disability and their carers by:

  • Identifying services and supports in your community that will assist you to reach your goals
  • Assisting to develop service agreements with service providers and obtain quotes for supports
  • Assisting you to coordinate services and complement support
  • Assisting you to resolve points of conflict with providers
  • Providing support, guidance and solutions in a time of crisis
  • Assisting you to source suitable accommodation and respite options
  • Working with you to develop your skills so you can confidently manage the NDIS plan over time
  • Monitoring your supports and providing reports back to the NDIA
  • Assisting you to prepare for plan review.

How do I access Support Coordination at Carers NSW?

Carers NSW is currently at capacity and unable to take new referrals for NDIS support coordination at this this time. We apologise for any inconvenience.

  • Step 1: Talk to your NDIA Planner or Local Area Coordinator (LAC) and discuss what level of Coordination of Support you would like to include in the NDIS plan
  • Step 2: Once Support Coordination is included in the plan, your NDIA Planner will provide you with a referral form
  • Step 3: Select Carers NSW as your NDIS Support Coordinator by completing your NDIA referral form and post it to Carers NSW (PO Box 785 North Sydney NSW 2059), email to contact@carersnsw.org.au or by phone
  • Step 4: We will contact you and arrange a time to meet to discuss your needs
